It’s important for DOD employees to report any hazards, injuries

With Hurricane Harvey’s rains and winds, there is an increase in hazards and risk for injuries.

Individuals are able to report new hazards with AF Form 457 to their respective safety office so they can be addressed and either removed or repaired, preventing possible injuries.

If a Department of Defense employee is injured, the injuries need to be reported to their safety office. There are forms that the individual is required to fill out in conjunction with their supervisor.

Military members:

  • Air Force Form 978 Supervisor's Mishap Report needs to be completed for any injury from minor to serious whether they occurred on- or off-duty. The document is then filed with the safety office.

Civilians:

  • CA-17 Duty Status Report Form needs to be completed for any injuries sustained in the performance of their duties prior to treatment unless it is an emergency. It authorizes the DOD to cover the costs for treatment to work-related injuries. The document is then filed with the hospital.

  • CA-1 Federal Employee's Notice of Traumatic Injury and Claim for Continuation of Pay/Compensation needs to be completed within 30 days of the injury. This allows the individual to report and document the injury, and file a claim. The document can be filed online following instructions at https://www.ecomp.dol.gov/content/help/HowToFile.html.
