Fill out short assessment form; briefly meet with technician, and then the triage provider. This service is to address immediate safety concerns and is short. You will then be given an appointment specific to your mental health needs.

For emergencies: Report to BAMC, Closest ER or Call 911

New Patient Evaluations: Due to high volume of patients, to prevent wait times up to 3 - 4 hours it is recommended you download new patient forms and bring completed forms to your appointment.

Deployment, PCS, Security & Job-Related Clearances: Please contact clinic to order outpatient record which is needed for clearances. (Records can take up to five duty days to receive. Please give us as much notice as possible.) Please bring all documentation pertaining to clearance to the clinic for review prior to your appointment.
